Practicefirst wants to help you stay informed and prepared when onboarding new providers.
Early planning is one of the most effective ways to avoid credentialing delays, enrollment backlogs, and billing interruptions once a provider starts. Having required documentation ready early allows credentialing and payer enrollment to move forward efficiently and helps support a smoother onboarding experience.
New Provider Onboarding Checklist
To help keep onboarding on track, practices should ensure the following items are available for each new provider:
Personal & Identification Documents
- Current CV (complete and up to date work history, in MM/YY format)
- Government‑issued photo ID (passport or driver’s license)
- Social Security number (securely maintained)
Education &Training
- Copies of degrees or diplomas
- Residency and/or fellowship completion certificates
- Training certificates relevant to specialty, if applicable
- Licensure & Certifications
Active state medical or professional license(s)
- DEA registration (if applicable)
- Controlled Substance Registration (if required by the state)
- Board certification(s), if applicable
- Insurance & Compliance
Professional liability (malpractice) insurance certificate
- NPI (National Provider Identifier)
- Medicare and Medicaid enrollment status (if applicable)
Credentialing & Enrollment Information
- CAQH profile completed and attested (provide username and password)
- Hospital privileges or admitting arrangements, if required
- Practice locations and contact details confirmed
Credentialing timelines can vary significantly by payer. While many carriers complete credentialing and enrollment in approximately 90–120 days, some may take up to six months or longer, depending on the payer, specialty, and application volume.
If a provider sees patients under a payer before credentialing is complete, those services may not be eligible for reimbursement. Planning ahead and initiating credentialing well before a provider’s start date can help reduce the risk of unpaid claims and revenue disruption.
